Coming in for an Appointment

Our offices are on the second floor of the building and accessible to older and disabled patients.

Office visits are by appointment. Please call us at 314.843.8000 for day and evening appointments.

If you are unable to keep your appointment, please notify us as soon as possible. We will be happy to reschedule you. Likewise, we will try to keep on schedule. Due to unforeseen emergencies, however, delays are sometimes unavoidable. We ask for your patience. If we anticipate a lengthy delay the day of your appointment, we will try to reschedule you.

What to Bring to Your Appointment

  • Your insurance card(s) and some form of ID (social security card or photo ID)
  • Physician referral forms if required by insurance
  • A list of current prescription or over-the-counter medications you are taking, including dose and frequency
  • Pertinent information about your medical and surgical history. Bring any pertinent x-rays or records you may have.

Patient Forms

The following patient information package can be printed out, completed and brought to your appointment:

Emergencies

A physician is always on-call for emergencies. If an emergency occurs during our regular work hours, call our office, 314.843.8000. This will allow the physician immediate access to your medical records.

If an emergency occurs after hours, call our emergency medical exchange number: 314.388.6530. A physician will contact you as soon as possible.

If, however, you need immediate medical treatment, go to the emergency room of a hospital where our physicians are on staff. The medical exchange can give you this information or click here for a list: hospital affiliations.

Non-Emergency Questions

Questions, directed to our offices, about your medical condition and treatment will initially be handled by a nurse and returned as soon as possible, unless they are a medical emergency. Non-emergency questions are returned later in the day, when the doctor is not seeing patients.

Payment and Insurance

Our fees are competitively priced, with payment for office visits due at the time service is rendered, unless your insurance carrier dictates otherwise. Personal checks, VISA, and MasterCard are acceptable forms of payment.

For your convenience, we will file your Medicare and Blue Cross/Blue Shield claims electronically via our computer system. This results in faster claims payment to you. If you are privately insured, you will be provided with the necessary form to submit your claim. This form should accompany other pertinent insurance papers and be mailed back to your carrier.

Please let us know if a second surgical opinion is required by your insurance carrier. Failure to do so could affect coverage of your surgery. Following your operation and after we have received your forms, we will file for surgical fees with your insurance carrier. Payment of all outstanding surgical charges is due in 90 days with the patient being responsible. If this presents a hardship, please contact our financial manager prior to surgery. Please remember, however, that your health insurance is a contract between you and your carrier.

A complimentary copy of our financial policy detailing this information will be given to you upon your initial visit.

If you have any questions regarding patient billing concerns, please call us at 314.843.8000.
