Medical Records Release

If you want a copy of your medical records, please print and complete the Medical Records Release Form. We cannot release any medical information about your diagnosis or treatment here without this authorization.

You can mail, fax or bring the completed form to our office. We recommend you deliver the form to office for timeliness’ sake.

We request a two-week notice to gather this information. If you need your medical records for an upcoming appointment that is less than two weeks away, please let us know immediately. We will do our best to comply.
