New Patient Forms

We encourage you to print and complete the patient information packet before you arrive. Otherwise, please plan to arrive at least 20 minutes before your scheduled appointment to complete necessary paperwork.

You must bring the following:

• Your insurance card(s) and some form of ID (social security card or photo ID)
• Physician referral forms if required by insurance
• A list of current prescription or over-the-counter medications you are taking, including the dose and frequency
• Pertinent information about your medical and surgical history. Bring any pertinent x-rays or records you may have.

Because our urologists are also surgeons, they sometimes run late. We will make every attempt to alert you if this occurs. Likewise, please inform our office as soon as possible if you cannot make your scheduled appointment.
